End-to-End Revenue Cycle

Revenue Cycle Management for Skilled Nursing Facilities

Most facilities do not have a billing problem. They have a revenue cycle with one weak link and nobody with the time to find it. We run the whole cycle — or the part that is breaking.

Why Cash Flow Problems Are Rarely a Billing Problem

A skilled nursing revenue cycle has roughly a dozen handoffs in it. Admission and payer setup. Insurance verification. Authorization tracking. The MDS that sets the rate. Triple check. Charge capture. Claim submission. Payment posting. Denials. A/R follow-up. Medicaid pending. Month-end close. Each one is a place where money stops moving.

In most facilities one or two people own all of it. That works until somebody takes vacation, a payer changes its rules, or census jumps. Then the weakest link gives, and because the symptoms all look the same — cash is down, DSO is up — it is genuinely hard to tell whether the cause was a verification miss in admissions, an MDS coding issue, or a claim nobody followed up on.

The usual response is to work harder on billing. But if the break is upstream, faster billing just produces faster denials.

What Our Revenue Cycle Management Includes

We can run the entire cycle, or step into the specific stage that is failing. Most engagements start narrow and widen once results show.

  • Admission and payer setup — census verification, payer assignment, and correct setup at the point of admission rather than at billing
  • Insurance verification and eligibility — benefits confirmed before the stay generates charges nobody can collect
  • Authorization and level-of-care tracking — managed care authorizations obtained and tracked through the stay
  • Triple-check coordination — clinical, therapy, and billing reconciled before claims go out
  • Charge capture and ancillary billing — therapy, pharmacy, and ancillary charges captured and billed correctly
  • Claim submission and clearinghouse management — clean claims out on schedule, rejections worked at the clearinghouse
  • Payment posting and cash application — remittances posted accurately so your aging reflects reality
  • Denial management and appeals — denials worked to root cause, appealed with documentation, and tracked to resolution
  • A/R follow-up by payer and bucket — systematic follow-up rather than whoever shouts loudest
  • Medicaid pending and application follow-up — applications tracked and retroactive eligibility billed once approved
  • Private pay and responsible party — statements and follow-up handled professionally
  • Month-end close and reporting — close on schedule, with DSO, aging, collection rate, and denial rate you can actually act on

Payers and Stages We Cover

What Makes NHBS Different

One team across the whole cycleOur MDS consultants, billers, and business office staff are the same organization. When an assessment causes a denial, nobody hands you between vendors.
Take all of it or part of itFull outsource, interim coverage during a vacancy, or a single stage that is broken. We scope to what you need, not to a package.
Skilled nursing onlySNF revenue cycle is not clinic billing with different codes. Coinsurance days, Medicaid pending, consolidated billing, and MDS-driven rates are routine here.
Reporting you can act onDSO, aging by bucket and payer, collection rate, and denial rate — delivered on a cadence, not on request.

What Changes When This Is Handled Properly

We do not publish a DSO figure on this page, because the honest answer depends on your payer mix, your census, and where your cycle is currently breaking. What changes is structural:

Cash becomes predictable

When every stage has an owner and a cadence, month-to-month collections stop swinging on who was available that week.

You can see where money stops

Reporting by stage and payer means a cash flow problem has a location, not just a symptom.

One accountable team

No triangulating between a billing vendor, an MDS consultant, and your business office when something breaks.

Revenue Cycle Management FAQs

What does full revenue cycle management actually include?

Everything from the point of admission to the point the money is collected and closed: payer setup, insurance verification, authorization tracking, triple check, charge capture, claim submission, payment posting, denial management, A/R follow-up, Medicaid pending, private pay, and month-end close.

Billing is one stage of that. Facilities that outsource only billing often find the problem was upstream.

Do you replace our business office?

Only if you want us to. The three common arrangements are full outsource, interim coverage while a position is vacant, and co-sourcing where we take specific stages and your staff keeps the rest. Co-sourcing is the most common.

How is this different from just outsourcing billing?

A billing service files claims. Revenue cycle management owns the outcome — including the verification that should have happened before admission and the denial that has to be appealed after. If your claims are clean but your cash is still down, billing was not the problem.

Can we start with only part of the cycle?

Yes, and we usually recommend it. Pick the stage that hurts most — denials, aged A/R, Medicaid pending, month-end close — and start there. Expanding later is straightforward because we are already in your systems.

Which software do you work in?

Yours. We work inside the clinical and billing platforms you already use rather than asking you to move data into ours.

How does this connect to your MDS and LTCMI services?

Directly, and this is the main reason facilities consolidate with us. The MDS sets the Medicare and Medicaid rate, and in Texas the LTCMI gates Medicaid payment entirely. A revenue cycle vendor who cannot see the assessment side is guessing about half of your reimbursement.
